7th Grade Immunizations
7th Grade Immunization Requirements
As we prepare for our return to school, the requirement of immunizations for all 7th graders: .
- Georgia state law requires that all seventh graders, born on or after January 1, 2002, must have a Meningitis (MCV4) and a Tetanus (TDAP) vaccine on a GA 3231 immunization record before entering the seventh grade.
Please contact or visit your local health department or private healthcare provider to verify that your child has already received both vaccines and records are updated in GRITS (Georgia Registry Immunization Transactions and Services).
When your child receives the vaccines at either the health department or private doctor, please have the doctor update your child’s immunization record online. The school clinic will verify all immunization on the online system. No form is needed at the school, but all 7th grade students will be verified through the online system. This immunization is a state law and required for all seventh graders to remain in school.
If you wish to seek an exemption, you must provide one of the following:
• GA Form 2208 (Affidavit of Religious Objection to Immunization)
• Medical exemption noted on GA Form 3231 by the physician.
Thank you for working to complete the immunizations before the first day of school.
All 7th grade students are required to have before September 1 to remain enrolled in school.
